Abstract

Juvenile gangrenous vasculitis of the scrotum is an unusual cause of scrotal ulcer endemic nature with a self-limited
and favourable development. We present the case of a 21-year-old man, whom in the context of pultaceous
pharyngotonsillitis, developed a single necrotic scrotal ulcer with elevated acute phase reactants, leukocytosis
with left deviation and image studies suggestive of Fournier's gangrene. We proceeded with urgent debridement, which was not compatible with the initial diagnosis. After reassessment of the case with the Internal Medicine Physician Comanagement Unit, the possibility of scrotal vasculitis was considered, which was confirmed by an histologic assessment.
