Lumbar spondylodiscitis, rare presentation of tophaceous gout

Microcrystalline arthropathy is a very common pathology in the general population, being the axial presentation a rare condition. We present the case of an older man with disabling low back pain which required a biopsy to reach a diagnosis of spinal gout despite a previous lumbar magnetic resonance imaging. After introducing the classic treatment there was a significant clinical improvement. Diagnosis is complicated, so it is essential to include clinical suspicion of chronic low back pain.
