We describe the case of a pluripathological and dependent elderly man who attends to Internal Medicine office. He complained about dizziness, sweating, digestive discomfort and anorexia, which led him to consult in Primary Care and Emergency Department. He denied therapeutic changes and referenced good adherence to the prescribed treatment. Through an exhaustive reconciliation of the medication, adherence failures, improperly prescribed drugs and possible medication side effects are detected, allowing the deprescription of some medications.
