Osteoarticular multifocal tuberculosis without lung disease

Osteoarticular tuberculosis is an uncommon form of extrapulmonary tuberculosis, representing a true diagnostic challenge due to its atypical presentation and its similarity to neoplastic osteolytic processes. We reported a 36-year-old male from Pakistan, immunocompetent without relevant pathological history, with chronic lumbar pain and a thoraco-abdomino-pelvic computed tomography with contrast, what showed multiple osteolytic lesions with a soft-tissue component. Puncture of the intramuscular collection of the sacral region was performed, with positive culture for Mycobacterium tuberculosis complex multisensible. The tuberculostatic treatment was started, achieving important clinical improvement.
