Abstract

Malignant rheumatoid arthritis is defined as the presence of severe extraarticular manifestations due to vasculitis. This usually occurs in males, smokers, with long-standing disease and marked activity. We present the case of a 52-year-old woman with rheumatoid arthritis in the first year of evolution who presented critical ischemia of both legs. Biopsy: vasculitis with endarteritis obliterans and panarteritis nodosa component. Her outcome was poor, requiring amputation despite anticoagulant, antiplatelet and treatment with corticosteroids, cyclophosphamide and the use of prostaglandin analogs.
