Abstract

Fibrinolytic therapy is the mainstay of the treatment of acute ischemic stroke. New embolic strokes as a complication of fibrinolytic treatment are described in a few cases in literature. It is believed that it could be due to the disintegration of a preexisting intracardiac thrombus. We present the case of a 63-year-old patient admitted for ischemic stroke of the left middle cerebral artery who undergoes fibrinolysis, after that she suffered multiple infarctions in different vascular territories, as consequence of a release of fragments of an intraventricular not-know thrombus. Early neurological deterioration after thrombolysis, with association of clinical signs corresponding to another vascular territory (different from the initially affected one), we should think about this entity.
