Thyrotoxic hypokalaemic periodic hypokalaemic hypokalaemic paralysis (HPTP): a metabolic emergency

Abstract

Thyrotoxic hypokalemic periodic paralysis (THPP) is a rare manifestation of hyperthyroidism, characterized by muscular pain, weakness and hypokalemia, which may lead to the death of the patient. Female 38-years-old, who consults the emergency with 24 hours of muscular pain and weakness. The clinical exam revealed severe paraparesia with tachycardia. The laboratory shows severe hypokalemia, decreased TSH and elevated free T4. The electrocardiogram has evidence of sinus tachycardia and flattened T. A diagnosis of THPP is performed. Potassium replacement and treatment with methimazole and propanolol was administered. The patient had improvement of symptoms and normalization of potassium.
