Abstract

Coronary or myocardial muscular bridging is a congenital anomaly that describes the intramyocardial segment of the coronary artery, called a tunneled artery. Myocardial bridging can occur in any artery, however, 67-98% occurs in the anterior descending artery. Diagnosis is usually incidental at angiography or autopsy; most individuals are unaware that they are carriers of this congenital anomaly until they present clinical manifestations that may suggest an acute coronary syndrome. We present the case of a patient with ST-elevation acute coronary syndrome who was incidentally diagnosed with myocardial bridging, successfully treated with calcium antagonist.
