Generalized weakness: hyponatremia or myasthenia?

Abstract

A 71-year-old woman came to the Emergency Department with asthenia and she was found to have moderate hyponatremia. The patient was admitted for correction and further evaluation, with an initial diagnosis of drug-induced SIADH. However, a chest CT scan revealed a mediastinal mass suggestive of thymic carcinoma. During the hospitalization, sodium correction was ineffective until oral urea was considered as a therapeutic option for treatment. She was discharged with a diagnosis of malignancy-associated SIADH, pending surgical evaluation. A few days later, the patient was admitted to Neurology, where her generalized weakness was diagnosed as myasthenia gravis associated with thymoma.
