A 56-year-old man with chronic pancreatitis was admitted for abdominal pain, vomiting, and weight loss of 10 kg. An abdominal computed tomography was performed, revealing the existence of a pancreatic pseudocyst, later complicated by a secondary pseudocyst infection, originally treated by endoscopic drainage and empirical antibiotic therapy. After an initial improvement, he develops a bacteremia by penicillin resistant Lactobacillus. Targeted antibiotic treatment was set up, with an improvement in clinical situation.
