A 73 year-old male, ex-smoker, with hypertension, rheumatoid arthritis and associated interstitial lung disease. He is undergoing chronic treatment with prednisone and methotrexate. He starts study because of wasting syndrome, poor control of blood pressure, and the discovery of metabolic alkalosis and hypokalemia. In additional tests we find signs of increased mineralocorticoid activity but suppressed levels of aldosterone and plasma renin. In addition, we find hypercortisolism with biochemical confirmation of ectopic Cushing syndrome. In parallel, we reach the diagnosis of oat-cell lung carcinoma as the neoplasia that justifies the paraneoplasic syndrome.
