Hyponatremia due to SIADH secondary to subarachnoid and intraparenchymal hemorrhage: role of urea in treatment

Abstract

Urea has demonstrated to be an effective tool to treat hyponatremia caused by syndrome of inappropriate antidiuretic hormone secretion (SIADH) when the cause is not initially reversible. We present the case of a patient who, after suffering a traumatic brain injury with intracranial hemorrhagic complications, began to experience confusion caused by hyponatremia secondary to SIADH. Due to the lack of response to fluid restriction we initiated treatment with urea, with an improvement of the symptoms and the natremia. After the urea was discontinued the clinical picture returned, so the treatment was reintroduced and the dosage was slowly decreased until removal.
