Cardiac involvement in seronegative immune-mediated necrotizing myopathy

We present the case of a 43-year-old patient with weakness in the lower limbs that had been going on for
months, associated with dysphagia and severe weight loss, with elevated serum creatine kinase and an electrophysiological study compatible with inflammatory myopathy, with a pathological diagnosis of immune-mediated necrotizing myopathy (IMNM). After starting treatment, the patient presented symptoms of acute heart
failure after entering atrial fibrillation, showing biventrivular dysfunction and data of myocardial infiltration on cardioMRI.
