Abstract

Ekbom syndrome is a psychiatric disorder in which the patient believes that is infested with bacteria, viruses, or worms.
It can be a primary or secondary disorder. We present a 72-year-old man with a delusion of infestation as the only
symptom, without an initial suspicion of an underlying organic condition due to normal physical examination and
blood test results showing no elevated markers of infection or inflammation. During admission, the patient was
transferred to Intensive Care due to shock, with an abdominal CT scan revealing findings consistent with acute cholecystitis
over chronic xanthogranulomatous cholecystitis. Infestation disorders are rare as secondary delusions, so it is
important to recognise that in elderly patients, they may be associated with hidden organic conditions.
