Revista Española de Casos Clínicos en Medicina Interna https://www.reccmi.com/RECCMI <p><strong>REVISTA ESPAÑOLA DE CASOS CLÍNICOS EN MEDICINA INTERNA (RECCMI)</strong></p> <p><strong>ISSN: 2530-0792 | Contact: </strong><a href="mailto:direccion@reccmi.com">direccion@reccmi.com</a></p> <p>The <strong>Revista Española de Casos Clínicos en Medicina Interna </strong>(Spanish Journal of Clinical Cases in Internal Medicine, or RECCMI according to the Spanish acronym) is a scientific journal published by the <a href="https://www.fesemi.org/">Sociedad Española de Medicina Interna</a> (SEMI) under its Board of Directors. The journal aims to improve clinical expertise surrounding Internal Medicine patients and provide specialists in training and those recently accredited (as well as all other clinicians) a forum for sharing specific experiences in the shape of their first or most recent work.</p> <p>The aim of the journal is to publish clinical cases and care tools capable of improving skills and training to enhance daily patient management.</p> <p>The journal is published <strong>every four months</strong>. The first issue covers the period from January to April, the second from May to August, and the third from September to December. National and international researchers write their publications in Spanish with abstracts in English; these publications are presented in an electronic format and are <strong>open access</strong>. Apalutamide is a selective inhibitor of androgen receptors used in cases of prostate cancer resistant to treatment with hormone blockade. There are three cases of drug-associated ILD published to date. We present a patient under treatment with apalutamide who developed a very rapid progression of ILD. The pancreas is affected in more than 50% of cases and the involvement of other organs together with the resolution of corticosteroid disease constitute distinctive features of this pathology. We present the case of a 53-year-old woman with abdominal pain, jaundice and submandibular tumors, elevated serum levels of IGG4, biopsy of the submandibular salivary glands compatible with fibrosis, images of the pancreas with enlargement and dilation of the bile ducts. However, in up to 1% it can manifest with dissemination to other organs, with cardiac involvement being extremely rare. Although it is still very rare, in the last decade, we are experiencing an increase in its incidence (0.68 per 100,000 population/years). We present the case of a 74-year-old man being followed up in the internal medicine day hospital for short bowel syndrome with the need for continuous administration of fluid therapy, hydroelectrolytes and parenteral nutrition. After an etiological study, a diagnosis of thrombotic microangiopathy was reached. Echocardiography is the diagnostic mainstay. There is no defined therapeutic regimen and treatment is based on surgical resection accompanied by chemotherapy and radiotherapy, with a poor prognosis. It affects mostly elderly people, and can manifest as cardiovascular disease, with cardiac arrhythmias. We present here the case of a 76 years old man who consulted due to fever. He had a history of two previous admissions for liver abscesses secondary to Klebsiella pneumoniae. The patient required treatment with pulses of methylprednisolone, rituximab and immunoglobulins due to severe involvement of the esophageal muscles and proximal extremities. During the 142 days of admission, the patient presented numerous complications with a slow response to the installed treatment.
